Prescription Refill

Please take a few moments to read our procedures so that you may fully understand the services offered by our pharmacy. Our online refill form below is strictly for compounded prescriptions. We recommend that you reorder your compounded prescription at least seven days prior to needing your medication. If you wish to request a refill on your conventional prescriptions, please call our 24-hour voicemail line at 715-854-7425; press option 1.

NEW PRESCRIPTIONS
Your licensed practitioner may phone in your new prescription order(s) by speaking directly to a pharmacist or leaving the prescription order on our 24-hour voicemail line at 715-854-7425, then press option 1 (to leave a voicemail) or 2 (to speak directly to the pharmacist).

A new prescription may also be mailed to Crivitz Pharmacy. When mailing in a prescription order, please provide the following information – please print clearly.

REFILL PRESCRIPTIONS
After receiving your initial prescription, we recommend that you reorder your prescription at least seven days prior to needing your medication. Doing this may prevent an unnecessary lapse in your therapy.

You may reorder your prescription by using any one of the methods shown below. All orders will require payment at the time of order. See “payment” below for more information.

SHIPPING
We offer FREE SHIPPING for compounded prescription orders within the US. Orders are shipped by US Postal Service Priority Mail®.

If you require a faster delivery method, we offer other expedited services. If expedited services are desired, please make that request within the comment section of your refill request form. Shipping charges may apply with other expedited services.

PAYMENT
Payment is required at the time of order.
Payment can be made by VISA, MASTERCARD or DISCOVER.
