 Ezra Gruszynski, Pharm D. / Owner
Ezra graduated with a Doctor of Pharmacy degree from the University of Wisconsin in 2001. He moved back to his hometown of Crivitz where he was employed at Crivitz Pharmacy as a staff pharmacist until 2006 when he and his wife, Missy, purchased the pharmacy. Since then, they have worked hard to turn Crivitz Pharmacy into a state-of-the art pharmacy which included adding a compounding lab in 2009 and opening Ranger Pharmacy in 2010. Ezra believes that each customer should have their expectations exceeded every time they visit either of the pharmacy locations. Ezra and Missy currently reside in the Crivitz area and are both actively involved in the upbringing of their three children.
 Brian Plucker, Pharm D. / Compounding Pharmacist
Brian graduated from the University of Wisconsin in 2005 as a Doctor of Pharmacy. He moved back to his hometown of Crivitz and began his career as a retail pharmacist at Crivitz Pharmacy in 2007. In 2009, Brian attended PCCA’s Primary Compounding Training in Houston, Texas. During this time, Brian completed in depth training on compounding pharmaceutical preparations. With this knowledge and continued education, Brian currently manages the compounding laboratory and takes pride in the preparations that compounding provides. Outside of the pharmacy, Brian enjoys time spent with his wife, Traci, and the outdoors.
 Holly Fayas, Pharm D.
Holly graduated with a Doctor of Pharmacy degree from the University of Wisconsin in 2010. She moved to Niagara, WI and began her employment with Crivitz Pharmacy immediately after graduation. Since then, she has progressed into lead pharmacist at our Ranger Pharmacy location. She applies her latest, most innovative pharmacy knowledge through patient education and internal expansions. Along with her commitment to her profession of pharmacy, Holly enjoys spending time with her husband, Justin, and their two dogs.
 Salina Polomis – Goodwill Ambassador
Salina has been a motivational part of our team since 2010. She has the passion and drive to promote the good health
and well-being of every patient. In person and through seminars, advertising and independent events, Salina is the spokeswoman and coordinator for the compounding and general education portions of Crivitz pharmacy. She has helped in the continuation of making our pharmacies the trusted resource for both patients and practitioners.
 Kathy Gilligan – Business Manager
Kathy has been an integral part of our team since 1998. She has progressed from her role as a pharmacy technician into her current position as Business Manager. By handling all corporate daily operations, she maintains the balance between both patients and employees.
 Judy Guth – CPhT, Insurance Billing
Judy has been a dynamic part of our team since 1998. She earned her pharmacy technician certification in 2007. Her desire for precision has helped make us a true “community healthcare center” for both patients and practitioners. Under her guidance, Crivitz Pharmacy has become an accredited provider of durable medical equipment. Judy is our patients’ resource for therapeutic shoe fittings, compression stocking measurements, and related insurance billings.
